Data Protection Policy

To provide services that are effective and safe, I will need to request certain personal information from you, some of which may be of a sensitive nature. I am committed to protecting any information that I collect in a way that is in compliance with the 2018 General Data Protection Regulation (GDPR). In order to practice in accordance with GDPR, I am required to inform you what Data will be requested from you, how that Data will be processed or shared, and what your rights are in relation to that Data.

The data I will hold for you may include:

  • Your name and contact information (including phone number, email address, and home address)

  • Your date of birth

  • Your child’s name and date of birth

  • Your partner’s name (if applicable)

  • Medical history (including obstetric and breastfeeding history)

  • Consultation notes, including plans

Any information that I gather from you will be:

  • used fairly, lawfully and transparently

  • used for specified, explicit purposes

  • used in a way that is adequate, relevant and limited to only what is necessary

  • accurate and, where necessary, kept up to date

  • kept for no longer than is necessary (I will retain your consultation data for a period of 25 years as required by law)

  • handled in a way that ensures appropriate security, including protection against unlawful or unauthorised processing, access, loss, destruction or damage

Your rights

Under the Data Protection Legislation, you have the following rights in regards to your personal information:

  • the right to be informed about the collection and the use of your personal data

  • the right to access personal data and supplementary information

  • the right to have inaccurate personal data rectified, or completed if it is incomplete

  • the right to erasure (to be forgotten) in certain circumstances

  • the right to restrict processing in certain circumstances

  • the right to data portability, which allows the data subject to obtain and reuse their personal data for their own purposes across different services

  • the right to object to processing in certain circumstances

  • rights in relation to automated decision making and profiling

  • the right to withdraw consent at any time (where relevant)

  • the right to complain to the Information Commissioner
